Maximizer (PPO)

The Maximizer plan is ideal for individuals who want broad coverage for a range of services, including pharmacy benefits. The Maximizer includes a $20 co-payment for doctor office visits, alternative care and urgent care centers.

Accident benefit Deductible waived for treatment completed within 90 days of accident.
